Card: Plastic The first of its kind, our official card for anyone awaiting assessment/ diagnosis. We understand the frustration for those whom are presenting as Neurodiverse but with the waiting lists being exceptionally long currently, can leave people feeling lost with a lack of understanding. Our new first of its kind Neurodiversity presenting card is to help families have something to prove presenting difficulties. This card can also help a young person/adult when in a meltdown seek medical help or attention. When someone has communication difficulties this card can help professionals access records as it includes photo, NHS number and full name along with diagnosis, it also includes an emergency contact so they have someone to call in case of an emergency.
This card also includes a QR code that directs them to the Website for validity.
Once purchased, we will require the following information:
A face on photo
D.O.B
NHS number (you can access this on NHS app or any letters/diagnosis
Their diagnosis/ proof of waiting list
Emergency name and contact details
